WELCOME TO OUR OFFICE
We are pleased to welcome you as a new patient to our office. We hope that this information will enable you to become familiar with our services and answer some questions you may have.

OFFICE HOURS
Our patient treatment hours are Monday through Wednesday 7:30 a.m. - 4:30 p.m. in Champaign and Thursday 7:30 a.m. - 4:30 p.m. in Mahomet.

APPOINTMENTS
Patients are seen by appointment only. Our schedule is designed to accommodate our patients and most importantly to run on time. It is impossible to see all of our patients after school; therefore we will do our best to accommodate your schedule. To avoid delays, please call at least 24 hours in advance of your appointment if you have loose or broken brackets. If you are unable to keep an appointment it is important to let us know as soon as possible. We do ask for 24 hours notice if an appointment needs to be cancelled. Rescheduling may result in a less desirable appointment time. Time is set aside each day to see patients with concerns.

FINANCIAL ARRANGEMENTS
We want your investment in a lifetime of beautiful smiles to work for you from the very first appointment. We will sit down with you and develop a customized payment plan that will work comfortably for you.

INSURANCE
If you have orthodontic insurance, we do accept assignment. However, in some cases we will be happy to provide you with the necessary paperwork for secondary and medical filing. Be assured we want you to maximize your full benefit.

PATIENT COOPERATION
Successful treatment is based on patient cooperation with appliances, elastic wear and oral hygiene. Please see your dentist every six months for cleanings.
